Advance Directives

 

A FREE Service of  Harbor Hospice

 

Every second Thursday from 10 till Noon questions can be answered regarding Advance Care Directives. To schedule a 30 minute session, call AgeWell at 733-8643.

 

You can pick up a free copy of the  Advance Directive document at Tanglewood Park, or Advance Directive document can be downloaded under the publications tab at the Senior Resources website:

Weekly:

  • Blood Pressure Checks: Every Monday from 9:30 am to noon in the Tanglewood Park Library.

  • Caregiver Support Group: Meets every Friday from 1-2:30 p.m. in the training rooms. Call 733-3585 for more info. Walk-ins are welcome!

Monthly:

  • Diabetes Support Group: Meets the last Friday of every month from 11am-Noon. Peer group led with lots of interesting statistics, encouragement, and sharing of recipes.

  • Hearing Clinic: Meets one time per quarter, call for appointment, 733-8643, 9 am-Noon in the Comerica Room.

  • Legal Advice: 30 minute appointments from 10am - Noon on the 1st and 3rd Thursday of every month. Call 733-8649 or stop at the Wellness Office to make an appointment.

  • Advance Directives: Second Thursday of every month, 10 a.m. to noon. Call 231-733-8643 to make an appointment to get help with your advance directive.

 

Programs/Classes:

 

  • Healthy Eating: A FREE hands-on program to learn about nutrition and how lifestyle changes can promote better health. Six week classes, call 733-8643 for class schedule.

  •  

  • The Savvy Caregiver: this FREE 6 week class uses the Savvy Caregiver curriculum to help family members caring for a loved one with dementia. Free respite care can be arranged.  For class schedules and to register call 733-3585.

  •  

  • Matter of Balance: (Managing Concerns about Falls) - FREE classes run one time a week for 8 weeks. Call for dates and to register at 733-8643.

  •  

  • PATH (Personal Action Towards Health): also called Chronic Disease Self Management Program - FREE classes run one time a week for 6 weeks. Call Virginia, for dates of next sessions at 231-683-2624.

  • Powerful Tools for Caregivers is a self-care education program for family caregivers. This 6-week program provides tools and strategies to better handle unique challenges caregivers face. Call 231-733-3585 for dates and to register for future sessions.
