AgeWell Services invites you to redefine age! We provide vital connections to keep you nourished, active, learning and living independently.
LifeCircles PACE program offers a personalized health and support program that helps eligible people live in the home and avoid moving into nursing facilities. The PACE program offers a hassle-free, one-stop-shop approach to senior care. It provides customized medical care, in-home care, nutritional support, rehabilitation therapies, socialization opportunities, day center programing, memory care, medical transportation, respite care for care partners and so much more to help seniors thrive in their homes and community. In addition to their formal PACE program, LifeCircles also offers a stand-alone, Adult Day Center program for those individuals who are not yet eligible for the full PACE program or are only looking for some day time supervision and engagement. LifeCircles offers two programs out of its
location at Tangelwood Park:
The PACE Program (Program of All-inclusive Care for the Elderly) and an Adult Day Center program.
As your Area Agency on Aging for Muskegon, Oceana, and Ottawa counties, we’ll listen to your unique life situation, present you with options, and help guide you to information and additional resources as needed. We want to help you navigate and access a wide variety of benefits and services for older adults and their caregivers. It may be eligiblity for government-supported programs, explaining application processes, helping with Medicare enrollment or changes, connecting with caregiver support services, or helping connect someone with needed services in their home. We have programs ranging from the MI Choice Waiver program to private pay programs, and everything in between.
